Become a member

Recruiting members in NewarkTo become a member you must be 16 years old and over and live locally in the areas specified below:

Ashfield – the area served by Ashfield District Council, together with selected wards from Gedling District Council

Mansfield – the area served by Mansfield District Council, together with Welbeck Ward from Bassetlaw District Council

Newark & Sherwood – the area served by Newark & Sherwood District Council and Rushcliffe District Council

Derbyshire – selected wards from Amber Valley and Bolsover District Councils

 If you live outside our local area, you can join as an affiliate member, giving you all the benefits of full membership, other than the opportunity to vote for a governor or stand for election to the Council of Governors.

If you are a member of another Foundation Trust you can still become a member of ours.  Our staff and volunteers are automatically members unless they have chosen to opt out.

There are a number of ways you can become a member:

  • Request an application form from the membership office by emailing your name and address to This email address is being protected from spambots. You need JavaScript enabled to view it.
  • Print the attached membership form and post to our Freepost address

Confirming your membership

After you have submitted your application form you will receive a welcome pack within 28 days.  This will include a member survey asking what you are interested in, for example finding out more about diabetes or cleanliness at the hospital.  You will then be contacted in the future if an event or group is arranged in your areas of interest.
