Discharge from Critical Care to the ward

We will arrange transfer to an appropriate ward when you are well enough, usually when you are no longer reliant upon the specialist support that critical care provides.  You will only be transferred following a thorough review by the critical care medical team.

Discharge from Critical Care to the ward is an important step forward in your recovery. You may feel anxious about your move to an area where there will be less medical and nursing intervention.

You may find it helpful to speak to the nurse about your fears.

The ward that you are transferred to will be dependent upon your medical needs.

 

Critical Care Outreach Team (CCOT)

When you are discharged from Critical Care, you will be referred to the Critical Care Outreach Team (CCOT). This means that a team of nurses who have a lot of critical care experience will follow you up on the ward to check that you are coping well without the higher level of critical care.

This follow up will usually start the day after you have been discharged and will continue for as long as required.
